On 2nd November the British Pregnancy Advisory Service (Merseyside) was rated inadequate by the CQC and placed into special measures. Reasons for the inadequate rating included: ‘Staff did not understand how and when to assess whether a patient had the capacity to make decisions about their care. We saw no evidence of mental capacity assessment for patients identified with cognitive impairment.’
