The Health Services Safety Investigations Body (HSSIB) have launched an investigation into concerns around the safety of people in mental health crisis who come into contact with urgent and emergency care services. The HSSIB is an independent arm’s length body of the Department of Health and Social Care. They investigate patient safety concerns across the NHS in England and in independent healthcare settings.

The investigation has been launched due to a range of concerns including Coroners reports into the deaths of people in mental health crisis in contact with urgent and emergency services.

The investigation formally starts spring next year but they want to hear from anybody with an interest in this subject matter now in preparation. This includes patients, families, carers and health and care professionals who may wish to share their experience.
