A useful report has been published by the law firm DAC Beachcroft analysing prevention of future death reports by Coroners during 2024.
The authors reviewed 330 Coroners reports and have grouped them into 4 areas - acute hospital, mental health, social care, ambulance. The report presents the most common recurring concerns from deaths in these areas.
