Coroner highlights failure to assess mental capacity


On 4th October, the Senior Coroner for Manchester City published a report on the death of Jude Daryl Lloyd who had chronic schizo-affective disorder and diabetes. He was admitted to a mental health NHS Trust and detained under Section 3. The coroner reported a number of concerns including: ‘During the course of the admission he refused to take his diabetic medication or agree to blood sugar testing. He denied having Diabetes or mental health problems...No formal mental capacity assessment was undertaken or recorded.’ He died of diabetic ketoacidosis at home after his discharge.
